Cancellation Policy: Bloom Medical Monthly Membership

At Bloom Medical, we are committed to providing you with exceptional healthcare services through our monthly membership program. To ensure clarity and transparency regarding the cancellation of your membership, we have established the following cancellation policy:


Monthly Membership Fee Charges:

  • Your monthly membership fee will be automatically charged on the same day each month, beginning from the date you initially enrolled in the program.


Cancellation Procedure:

  • 1. If you wish to cancel your membership, you have two options:

    • Visit your Vagaro account and follow the provided cancellation process.

    • Contact our  Bloom Medical staff by texting 801-899-6156.


Cancellation Deadline:

  • To successfully cancel your membership, you must initiate the cancellation at least 2 weeks prior to your monthly renewal date. This ensures that you won't be charged for the upcoming month.


Important Note:

  • We regret to inform you that we are unable to accept cancellations after the 2-week deadline has passed. This policy is in place because the monthly fee includes the cost of nurse monitoring services, which are reserved in advance to ensure your continued care and support.


Refund Policy:

  • Please be aware that if you decide to cancel your membership after your card has already been charged for the upcoming month, we are unable to provide refunds for that specific month's fee. We encourage you to plan your cancellations in advance to avoid any unwanted charges.


We appreciate your understanding of our cancellation policy, as it helps us maintain the quality of our healthcare services and ensures the availability of our skilled nursing staff 

Compliance Policy:

To minimize the risk of adverse events, the medication dosing schedule must be adhered to unless under specific guidance by your healthcare provider. Titrating the medication slowly is necessary to avoid severe side effects. Do not attempt to increase your dose ahead of the outlined schedule. Doing so may result in the termination of your membership with Bloom Medical, as we take medication compliance very seriously.
